For adult patients between the ages of 18-65 diagnosed with Type 2 diabetes and a BMI greater than 30, does the implementation of the ADA 2021 Guideline 5, compared to current practice, impact serum blood glucose and BMI in 10-12 weeks? Discuss best practices for implementing evidence-based practice/ guidelines with references sufficient to support best practice. Discuss strategies for overcoming barriers in implementing evidence-based practice with references sufficient to support strategies. Discuss your ongoing implementation progress and in meeting with staff and observation how will you plan to sustain your project and disseminate? Describe your sustainability plan: o Include how you would maintain the practice change? o Are champions appointed to assure sustainability? o Does the site plan to implement this project on other units? o Are there required changes to technology needed in order to sustain your project?
